What are some of the events leading up to Macbeth's death?

I am writing an essay on William Shakespeare's Macbeth and I need two specific instances that lead up to Macbeth's death [that can be proven by quotes]. Preferably in acts 1 thru 4. I have already noted that Lady Macbeth and her son has been killed. Does anyone else have anything else? THANKS!
